Proctocolectomy with an ileostomy and later on a J-pouch surgery?

I have been suffering from chronic ulcerative colitis (UC) over the last twenty years. Iím in control of it and lead almost a normal life.

Iím currently on 3 grams of Rafassal (Mesalazine) and 200 mg of Imuran (Azathioprine).

My last colonoscopy revealed multiple areas suspicious of low grade dysplasia (LGD) in sigmoid colon and recotoanal junction. There is also doubt they might be high grade dysplasia (HGD).

My doctor advised that these LGDs be removed. However, he also strongly recommended that the whole colon be removed (total proctocolectomy) with an ileostomy and later on a J-pouch be constructed.

My questions:

Is surgery my only option?

Would cleaning LGDs lower or eliminate the need for these operations?

Any natural alternatives to avoid it like garlic and also to prevent cancer from developing?

Thank you for sharing any experience, advice or recommendation.
