Question Is the Dr. questionare a biig part of SS making their decision?

I applied for disability May of 07.Since then I had a 3 level ACDF.I continue to have a great deal of pain as well as numbness and weekness in arms and hands.I also have a lot of pain in lower back and hips not yet diagnosed. Ive been denied twice and have now since hired a disability advocate.Since having my surgery Ive been going to pain management.Will that be the Dr. that should fill out my questionare?He really doesnt know much about my limitations only my pain issues.How will he fill out the questionare so that it is to my benefit?My NS so much as wrote me off he says he didnt do the surgery for pain relief but for nuerological reasons.Who should fill out my questionare and is it absolutely nessesary for my case? Is there anyone with a simular case ?
