About to lose my mind!

I am 33 year old, female, I have 3 children all healthy. I never had weight issues, nor trouble loosing weight until my 2nd pregnancy.

With that being said, all was really fine until last year when I started a constannt period that has not stopped! I thought I would give it time afte I had my 20 month old baby to let my body "heal" into its natural cycle. Instead this has happend:
a month after I had her I lost 40lbs (weight being 275 down to 245) then a month or two later I was down to 210.
I continued the "diabetic diet" that I was on from gestational diabetes with her, loads of exercise, etc. Well, by January (I had her in August 2009) I hit what I thought was a platau, no more weight was coming off, by febuary I went to a weight loss clinic, bought adipex and well after the summer I lost NO weight. Well about 6 months ago, I went to the dr because of having a constant period, and no relief. The doctor said it was "stress" and put me on progesterone. Well I have to admit, it worked for a couple months, then she doubled the dose, and I haven't stopped bleeding since.
So I saw a gyn doc, she suggested thyroid issues, and hormones, etc ordered a bunch of test:
My thyroid levels:
tsh 4.79
free t4- 1.0
no free t3 drawn

I failed the 2 hour glucose by it being 147, normal being 140

my 17-hydroxyprogesterone was 7
testosterone was 25
estrogen I can't remember

She placed me on birth control (still no help)
I go back to see her Monday and I am hoping for a clue as to what the heck all of this is.

I do have awlful pains that hit me (around my ovaries) about once a week that are bad enough to cause nasea, and have caused me to throw up, with those massive bleeding (soaking a tampon and thick pad in an hour).

I am weak, tired, have dry skin, hair is falling out, cannot loose weight, moody, depressed...

She mentioned pcos, and will do an ultrasound monday, and look at the lab work.

I also have a hystory of fat pockets in my cervical region and nodules on my thyroid.

Any help would be appreciated!!

btw my progesterone according to the last doctor was low, but I don't know the exact value
