Carotid Artery Ultrasound Conclusion Seems Wrong
When I compare my carotid artery ultrasound results to various Criteria for the Diagnosis of Internal Carotid Artery (ICA) Stenosis, I seem to fall into the category of occlusion, yet my report says normal. I am copying my results and the guidlines. Am I wrong or is the report wrong? What am I missing? MY REsults:
CHU 0033 - US CAROTID BILAT B PROCEDURE REASON: NEAR SYNCOPE
No significant plaque is visualized TECHNIQUE: Real-time imaging of the extracranial circulation was performed utilizing grayscale, color Doppler and pulse Doppler.
RESULT: ICA Peak Systolic Velocity (cm/sec) Right 133.3 Left 113.6
ICA End Diastolic Velocity (cm/sec) Right 19.2 Left 43.9
CCA Peak Systolic Velocity (cm/sec) Right 108.2 Left 102
ECA Peak Systolic Velocity (cm/sec) Right 80.6 Left 83.8
Vertebrals
Right antegrade Left antegrade
ICA/CCA Systolic Velocity Ratio Right 1.2 Left 1.1
ICA Stenosis Estimation - includes measurement of velocities in the right
and left distal common carotid arteries.
Right less than 30% per the NASCET criteria
Left less than 30% per the NASCET criteria
IMPRESSION: No hemodynamically significant stenosis is seen at the internal carotid arteries.
Here are the Guidelines with my numbers (in parenthesis).
Diameter StenosisPeak Sys Vel End Diastolic Vel Systol Vel Rati Dias Vel Ratio
0% <110 <40 <1.8 <2.6
1-39% <110 <40 <1.8 <2.6
40-59% <130 (113) <40 (19) <1.8 (1.2)(1.1) <2.6 (.17)(.43)
60-79% >130 (133) >40 (43) >1.8 >2.6
80-99% >250 >100 >3.7 >5.5
Seems like my numbers (in parenthesis)are either 40-59% blocked or 60-79%. Can anyone help? I hate to make a fool out of myself challenging the Dr who wrote the report! Thanks
