Taking Crestor for my high LDL. I have a '?' about side effects for high bilirubin.

I went to my doctor today to discuss my lab results for my high cholesterol. As always, my tests showed high LDL (170) while my HDH was 43 and triglycerides were 96. I'm 33 and have been testing for high cholesterol since I was a teenager, so it's no surprise. My bilirubin has also always tested high for well over 10 years since I've been paying attention to that test.

I figured it was about time I get on medications for my high cholesterol, and so my doctor today gave me Crestor. I've read the side effects and it says 'Do not take if you have liver problems or liver disease'. Well I don't have any known problems or a disease, but as far as I know have something called Gilbert's Syndrome (for the consistently high bilirubin), so I'm concerned whether or not I should be on Crestor or not and maybe something else.

Any comments would be great to hear from any of you.

    ASU,

Have you had any jaundice at all or is the Gilbert's totally benign? Does it run in your family?
Are you positive for Hepatitis B, C...or others?

Truth be told, in your place I wouldn't take any statin, least of all Crestor (the strongest,) without the ok of a hepatic specialist...especially since Gilbert's is made by exclusion.

Is it possible to talk your doctor into Zocor, which is milder.
Make sure if you start the statin to get frequent liver enzyme tests. Maybe monthly for a couple months.

Re: Crestor question side effects to treat my high LDL

    Lenin,
The whites of my eyes are jaundiced and have always been since I was a teenager as one friend noticed back in high school. I'm 33 now and have known about the whites of my eyes since about 1987. It was in 1994 that I first learned about my bilirubin being out the normal range. My skin has never been jaundiced, I don't have any known Hepatitis issues, and my family doens't have liver issues or Gilberts Syndrome, although I should ask them about their past bilirubin tests because I don't actually know what my parents have tested.

Of course when I visit my doctor (which is rare I do go) but when I do I just sit there and say ok to whatever he says. I wanted to ask him to put me on Lipitor but he gave me 5 weeks of Crestor samples and said we'll run blood tests (Liver Profile and Lipid Panel) in that 5th week. I wanted to ask why not Lipitor, mainly because its been around longer than Crestor and I believe there has been no known issues with Lipitor to Liver problems, but I don't speak up at times and question my doctor.

I have a very good friend who's a doctor so natually I talk to him about things and he was a little annoyed why my doctor didn't do Lipitor or Zocor. I just took my first Crestor Thursday night 3/24/05 so we'll see. I should go to a different doctor I suppose for a second opinion and see about a liver enzyme test and further analysis.

When and if I do, I'll update you with what I know. Thanks for the reply.

    Nick

    Nick,

Have that blood test look for antibodies to all the strains of Hepatitis
in that Fifth week, and tell your doctor precisely why. If you want Lipitor or Zocor, ask for it and make sure he gives you a good reason for taking out the big gun like Crestor. Your lipid numbers aren't bad enough to need the heavy cannons. ANything more than 10 mg. Lipitor or 20 mg Zocor is overkill.
Remember though, the BIG pills cost the same as the small ones and a pill splitter is VERY cheap.
What dose Crestor did he give you for your 5 week trial?

Pharm Rep samples of 5mg pills. Seven pills in a pack, Five packs ...So five weeks of pills.

I'll return to this post in the next couple weeks should I have a side effect (basically the only one I'll notice would be achy muscles). As for any liver issues, I'll know more once I get my lab test next month as I don't think I'll be feeling anything there.

    Thanks for your replies.

Within a week of taking Crestor I developed severe leg pain. The pain was
so bad I could hardly walk. I discontinued it at that point and the pain dissipated. Most people don't have that reaction to Crestor but it is something to keep in mind. Good luck!

    Nick,

Just to set the record straight, ALL STATINS POSE A RISK OF LIVER PROBLEMS.

That's why any doctor who prescribes them most usually does a first blood workup at the 3 month interval - to make sure the liver is tolerating the drug, and to see how the drug is working for the patient. There should be regular blood workups after this initial one -usually every 6 months.

From what I've read, the risk of liver damage is worse with Crestor than with other statins - it appears to be much stronger than the others.

I agree with Lenin - you should get a 2nd opinion to ensure that a statin won't cause liver problems since you say you have Gilbert's....

You have to be cautious... Some doctors are prescribing statins to anyone with a heartbeat - which is why they have a stethescope around their neck all the time...

Thanks Hub. I'll be taking my 5th pill tonight so I'm soon to be one week on this stuff. My next blood work is during my 5th week, so I'll know for sure by then if I really should be on this stuff or not. I'll keep you posted.

    ASU,

    Good luck!!!

If it gives you good reduction in LDL without any side-effects, I'd stay on it...

But be on guard for any adverse effects. You should be fine, but sometimes the problems develope after years on the drugs, so be careful... Consider taking CoQ10 to reduce the chance of muscle problems....

Update for Hubble and Lenin ...I've been on the Crestor two weeks to the day now and while I only am taking a small dose (5mg), I have no muscle pain or issues. I'll know more when I go back for my first lab test in three more weeks to check my cholesterol and liver profile.

Nick, I noticed that you said that you had questions you wanted to ask your doctor, but neglected to do so, just accepting his samples. You may find it helpful to write out your questions on a notepad before you go and leave it in your lap as you talk with the doctor. At an appropriate point in the consultation, you can say, "Oh, I have some questions." Then go down the list, very obviously pointing to each item as you begin it, so it is clear you have more. This may give you a little more feeling of control. It also helps those of us with a bad memory or who have doctors who want to do all the talking, then shove us out the door.

I've said this to others, and I'll say it again. (Some here are probably tired of reading it!) YOU, not the doctor, are the one in charge of your health. When you go to a doctor, you are essentially hiring him as your consultant, just as a business manager would hire an expert in another field. You may very well accept his advice, because he is supposed to be an expert. But he is not infallible. Ultimately, YOU are the manager responsible for the decision of what to do with that advice. If you accept that premise, it may be easier to be assertive in your consultations.

Re: Crestor question side effects to treat my high LDL

Yeah I'll be sure to ask him next time I see him ...which will be sometime after I get my next lab results back so early in May sometime. I don't ask because I tend to think he knows what he's talking about, and then I clam up and keep my mouth shut when I'm in front of him. I'll have to speak up when I see him, and that is a good thing to do is make a note because if I have 4 or 5 questions, I often might forget what 1 or 2 of them were when I get in there. Thanks.

Re: Crestor question side effects to treat my high LDL

Follow up for all who replied to my post. Especially Lenin and HubbleRules, - thanks for replies in the past.

Well I went to the lab after five weeks of taking 5mg of Crestor. Here's the breakdown as I just found out today. My first lab visit was about six weeks ago in mid-March. I went back last Friday on April 29th. Hereís the new numbers after five weeks of 5mg of Crestor.

    Total - 148 (228 six weeks ago)
    LDL - 87 (170 six weeks ago)
    HDL - 45 (43 six weeks ago)
Tri - 99 (I canít recall but I think it was at 97 six weeks ago)

I forgot to ask about the ratio.
Anyway I'm now getting a prescription for 5mg of Crestor since it seems to be working and I have No Side Effects. So for now I'll be on it and go back in six months for another lab visit. I haven't changed my diet either ...I'm on the go at times and will do the fast food if necessary, but generally I eat very well ...so I think with a stricter diet and regular cardio at the gym will only help keep it lower. I suffer from hypoglycemia (Low Blood Sugar for those of you who don't know), so sometimes when I need to eat I simply eat whatever (hence the fast food!). Anyway there's your Nick update. Thanks to all.

    Nick
