splenic calcifications with prior granulomatous disease

I am new here. I have had abdominal and right flank pain for a month. My father died of kidney cancer at 52 and my mother had bladder cancer and lung cancer and died from this. I am 59 yrs old. I have 4 plus pitting edema and concerned about my renal situation. I had a CT scan and it showed vascular calcifications of aortoiliac tree and multiple splenic calcifications compatible with granulomatous disease and renal cortical thinning/ scarring left upper pole with lobulation of both kidneys and enlarged appendix.I am very swollen and have pulmonary hypertention. Do any of you have this and should I be concerned? What kind of specialist should I see?
