Epoxy Cement in Hip Replacements

Less than four years after a cemented hip replacement the stem has loosened in the thigh bone, resulting in an upcoming re-replacement.
I heard that there was an alert about a "bad" batch of cement in the past few years. Anyone have details of this. Also any advice about the re-replacement.
