    Misshapen nail, extreme cuticle pain

    I've had a problem with my middle finger nail for about 5-6 years and I've seen 6 Doctors for it so far. 4 of them were hand surgeons, 1 dermatologist and 1 dermatological nail specialist. I've had an x-ray, MRI and cultures done. One dermatologist said I may have incurred some trauma to the finger or nail, resulting in some swelling under the nail bed. He cultured it and did not find any bacteria or fungus present. He gave me an anti-inflammatory cream to use occasionally which helped with the swelling but did not help with the finger nail shape or extreme pain I feel when anything brushes near the cuticle. One of the hand surgeons treated it as a paronychia, so I soaked it in warm water and epsom salts for over a year. None of this helped as it's continued to get worse over time and the pain response is completely disproportionate to the stimulus. I can't even reach inside my purse with that hand because if something touches it in the right spot, I feel as though it's been smashed by a hammer. It's not uncommon for me to hit it unexpectedly and be brought to my knees by the pain; it's so severe, it instantly makes me sick to my stomach. I get regular manicures but my nail tech takes special care of that nail in particular, treating it with extreme caution. She has kept a linen wrap on it for protection; however, after recently removing the wrap, I can see that the nail has gotten worse. So the issue is really three-fold: I do not know what's caused the swelling under the nail bed, the nail is growing in an odd shape and the pain outside my cuticle is unbearable. I've tried everything for the "common" nail irregularities and nothing has helped. Any ideas?

    I would certainly be concerned about an abnormal growth under the nail bed. This could be a simple cyst which can arise from arthritis in the joint next to the nail, or a bony growth from the distal phalanx. The misshapen nail and severe pain indicates pressure under the nail plate. Your finger should be xrayed and perhaps ultrsounded to look for an underlying growth., which could be surgically removed if found. Please go back to one of the hand surgeons, if you liked any one of them, and ask for those two tests. Some keep ultrasound, xray equipment in their office for quick assessments like this.

    Thank you very much, that's one thing I hadn't thought of or had not yet been suggested.
