    Methotrexate question

    My wife and I found out we were pregnant in February of this year. It was an unbelievable time in our lives together as newlyweds. About three weeks into the pregnancy, my wife experienced severe cramping and spotting. The on-call doctor thought that it was fishy and sent her for an ultrasound. The ultrasound showed that the sac was not in the proper location in the uterus and we were told that it could be a cornial pregnancy. After weekly visits to the high-risk specialist for 5 weeks, on April 11, the pregnancy was deemed to be cornial and had to be terminated immediately. A cornial pregnancy is a form of ectopic where the egg embeds in the uterus, but very close to the fallopian tube opening. When the sac gets big enough, it will rupture the uterus possiby causing my wife to bleed to death, but at the very least it would have led to a hysterectomy. So the decision was the easiest, hard decision we ever had to make. At the point of her pregnancy, her HCG levels were at 100,000.

    On April 12, she received a shot of KCL, which stopped the heart, and a shot of methotrexate, which is supposed to cause the sac and embryo to be absorbed back into the body. She could not have had a DNC or DNE because it was too risky and in a very unreachable area of the uterus. About three weeks ago, my wife passed the sac after a bought of severe cramping. An ultrasound revealed that she passed all of the tissue and her uterus was clear. The passing of the sac was so bittersweet. It was very hard to see the tissue itself, but at the same time, it marked a new starting point for us and allowed us to start moving forward. Her HCG levels have since dropped to 116.

    The hardest part throughout this entire process has been the unanswered questions. Cornial pregnancies are very rare and not much is known about them. We had so many questions where the answer was “I don’t know”. We both realize that we cannot fault the doctors for not having experience with our situation, but that does not help us at all.

    Both of us want so desperately to have a child, so we are very eager to try again as soon as it is safe for both mom and child. This is where we are receiving conflicting information, which is very frustrating. Her OB tells her that it is okay resume taking her prenatal vitamins and ttc after her first cycle. My wife is a part of an ectopic trust, and they are telling her to wait at least 3 months before she takes her vitamins and tries to conceive. They say that because methotrexate robs the body of it’s entire folate reserve and as we all know, folate is crucial for cell division and neurological growth. Who’s advice should we listen to? The people at the ectopic trust seem to have more answers and more experience with methotrexate, but they are not her OB.

    Does anyone out there know anything about methotrexate and what time table we should follow working toward getting pregnant again?

    My beautiful wife has been so strong and brave during this whole ordeal. I have tried to be as supportive as I can, but it honestly kills me that I cannot fix what ails her, or answer her questions or protect her from what frustrates her. I wish I could.
