Hip spasm that causes cramps that led to toes BREAKING!?

Asking for a friend: She’s 45, has had 6 kids (youngest is 5), female, doesn’t smoke, no medications, 5’ 9” and appropriately 200bs. -Saw an orthopedic doctor. X ray showed nothing wrong with joint or bones in affected hip. Just mild degenerative disk disease between L5 and S1. Said “IDK I’m kind of perplexed. I really only do hip replacements... I can prescribe you some Lyrica.” She asked about getting an MRI, he said it probably isn’t bursitis or a muscle tear, so he didn’t write her an order for an MRI.

About a year ago she started having some mild hip pain in the front of her hip. The pain does/did have an epicenter that hurts pretty badly in the hip when pushed on. Suspected bursitis. Maybe a muscle tear. Wasn’t sure because she could move her leg in any direction and it didn’t worsen or aggravate the pain. Only would hurt worse when pushed right on the spot.

Fast forward a year later, starts having some numbness around the hip area and weird nerve-y sensations in her upper thigh. This progressed until 2 weeks ago, she had a HORRIFIC episode that lasted for about 45 seconds to 1 minute long where her upper thigh muscle cramps up like someone with tetanus along with the the index and middle toes on the foot of the affected (right) leg. They cramp SO hard. Curling under her foot. She is also having near constant numbness and weird nerve sensations down her leg all day, every day. But during these episodes, the area around her knee starts to go numb and is followed by the cramps and her WHOLE leg is an engulfed in an unbearable form of weird, indescribable nerve sensation. In the ballpark of your leg completely going to sleep and then when it starts to wake up, but a million times more intense.

These episodes happened once a day, usually in the mornings about an hour or two after waking up. (Just to reiterate: she has weird nerve misfirings along with some numbness throughout the day, but the horrible episodes last a minute or so.) Well it has progressed to the point where the two toes that are affected during the episodes BROKE because she could pull them back with her hands in time.

WHAT COULD THE ISSUE BE????? Any ideas or leads would be helpful. She’s going to see her regular PCP on Dec. 13.

Sounds like she needs to see a neuro and have an MRI. Sounds like neuropathy to me caused by some pressure on nerves in her back from the disc issue. I have some similar pains and have 2 desicated discs, 2 bulging ones and scoliosis.

Thanks so much for your reply! She’ll be pushing for a referral to a neuro at her PCP.

Maybe her PC will order an MRI. I've been going to PT for mine and that really has helped so that may be all she needs.
