    Question Seeking advice on changing analgesic medication

    Hi guys...

    This is my first post, and it's pretty much the reason I'm here...

    My doctor recently referred me to a chronic pain specialist, however the recent COVID-19 pandemic has forced my pain management team to postpone my appointment until further notice.

    My GP/Primary Physician has referred me to the pain specialist, as he is running out of options, and is unsure of what he can do to help me... I have used Morphine (modified and immediate release preparations) for almost 5 years; during which time my doctor has tried increasing the dosage, however it seems to have little effect. Any improvements from increasing the dose are short-lived, usually becoming ineffective around 14-21 days after the dose was increased.

    I have asked if he would try changing my analgesic (pain killer), however he is uncomfortable making this decision without first seeking the advice from a chronic pain specialist.

    The conditions from which I suffer, cause severe chronic pain, to such an extent that my GP rarely has to treat such a patient for pain which is 'non-cancer' in origin, and therefore there are limitations in how high the dosage can be allowed to reach.

    Since the Coronavirus is now causing such an immense challenge to healthcare services on a global scale, there are very few options available to my doctor, with regards to any help he can offer. In my last conversation, he informed me that, until further notice, the only help available to me, is medication.

    For this reason, I am seeking the advice of others in similar situations to my own, with regards to which analgesics they have found most effective, so that I might ask my doctor to try a temporary change in treatments. At the moment, I am taking a total of 'Morphine Sulphate' 140mg - 180mg daily; in both immediate and controlled release preparations.

    Before I ask my doctor to try changing my pain relief, I would like to first learn more about which treatments others have tried, so that I might make some suggestions of which treatments could be more beneficial, than those I currently use.

    The primary reason for my doctor referring to a pain management specialist, is that I seem to have an unusually high tolerance to almost all medications which affect the central nervous system, whether using a CNS depressant, or CNS stimulant. This makes my doctor uncomfortable, as he expects the treatments he prescribes to have much more of an effect on me, whether the effect is desired, or a side-effect.

    If you have any information which might be of use to me, I would ask that you PLEASE offer the benefit of your knowledge, as I have nowhere else to turn, and there is nothing more I can do!

    Even before covid19 provided an escape hatch for pain treatment by medics, I have seen many posts in many places about your, and my, problem. Injections, OTC medications lacking in results are many, and gabapentin is no solution despite seeming relief initially. If we do the best under the circumstances, it seems to me the medical community has no answers without GREAT risks. In my case, my advantage is greater age, that assures me the solution will invest itself in my demise, so my suffering is perhaps nearer than yours.

    I am very sorry you are going through this! The pandemic makes everything scarier in my opinion. Have you at least been able to get an appointment yet? My PM was closed for 3 months, but we did Telehealth appointments so there was no break in my ability to get my meds. As a new patient you will have to have labs run, etc .. but hopefully you will get one of the great docs who really cares about his patients!
