Put on oxcontin with percocet for breakthrough, when to take the percocet?

Had appointment with Rehab. doc, he put me on oxycontin 20 mg cr (generic oxycodone 20 mg ER) every 12 hrs, he let me know I am the only patient he has on this & is doing it to help me get through this pain which is out of control until I see a specialist. He told me to use perocet as a breakthrough med along with the oxy. The percocet is 10mg, he never specified when or if there was a certain time frame to take the percocet, I asked the pharmacist & he was also vague & said just take it as a break through med, of course I am clueless as what is the best way to take these together. I started the oxy & it has taken some of the edge off but still in alot of pain, the percocet seems to work better then the oxy but does not last longer then 2 to 3 hrs at most lately. So should I take the perocet like at a half way mark in between? I am assuming it is only 1 percocet a day. I know I can half the percocet if needed, anyone know if I can split the dose in between the oxy doses? All these meds make me alittle nervous but I definantly don't want to be in pain. Also can I still take some motrin if I need to? I realize the pharmacist knows best but heck can't get a straight answer & the doc said he does not prescibe this often at all. I have an appointment the end of the month with a specialist & another MRI this thursday, I just need to hang on till I can see this specialist & am praying he can help me out, I already know I am going to hear I should not be on these meds, but I just can't take the pain & it is way out of control, so please any advice as always would be helpful. Thanks Sammy

Re: Put on oxcontin with percocet for breakthrough, when to take the percocet?

First of all you aren't on strong enough Oxycodone and that is why no one can give you a straight answer. Your doctor put you on a dosage and time schedule that is doomed from the beginning. What is SUPPOSE to happen is your doctor should have you on a more reasonable higher dose and a 6hour schedule and 20 mg every 12 hours is stupid and if you have no relief from it at all then there is no 'break through' pain because your pain was never alleviated in the first place. Break through pain is when you have pain in between the 12 hours and everyone is different so hard for anyone to give you advice but my guess is the pharmacist is probably wondering why doctor put you on such a small dosage of Oxy in the first place. If the dosage you were given doesn't totally stop the pain then you are not having breakthrough pain...because the pain never went away. I would call the doctor back and tell him that the Oxy dosage is not stopping the pain so what would he suggest? I'm not really sure why your doctor has you on two different drugs when he could keep you on one or the other just at higher dosages every 4-6hours...other than your doctor is afraid of the Feds(DEC). I suspect he is not going to change the dosage of the oxy because doctor is afraid so you are going to have to experiment with what you do have. You can take the two(Oxy and Percocet) together and use motrin...(up to 800) between times. If the two narcotics take the pain away then when you start having pain and 12 hours isn't up then that is what is called breakthrough pain. It is sorry excuse for pain meds and I am sorry your doctor is not really treating you properly...you are going to suffer and that is just not right. Allllll doctors know how to treat your pain but they are soooo afraid of the Feds that they are making the patient suffer and in your case your doctor claims to be doing you a favor until you see the pain clinic but he is not helping you at all..he is watching out for himself. In reality if doctor is treating the patient with the correct dosage then the feds don't care but it just gives the doctor more paperwork and explaining to do.

Re: Put on oxcontin with percocet for breakthrough, when to take the percocet?

I had spine surgery 11/28. I was put on oxycontin 20 mg. ER 2 x daily with 10-325 mg. percocet 1 tablet every 4 to 6 hours. Hope this helps you some.

My surgeon wants me off the oxycontin and I just want to laugh in his face. With this mixture of meds my pain has not been lower than a 5 since surgery.

Sammy, if it was me and I could not get a clear cut answer from my doctor. I would try to take the percocet every 6 hours. If you take it continious for 24 hrs, then you can determine if you can stretch it out 8 hours. If this does not reduce your pain, then I would give your doctor another call and ask if you should take it every 4 hours.

What does the percocet bottle say?

Re: Put on oxcontin with percocet for breakthrough, when to take the percocet?

    Sammy,

Keep a pain diary. And enter your pain level by the hour if necessary. Use the 0-10 scale where 0 = no pain and 10 equals the worst pain you can imagine. Also indicate in your pain diary when you take each medication, including those not prescribed, like Motrin.

Docs tend to work more aggressively with patients that use this approach.

The pain meds are not intended to kill all the pain. Rather, they are intended to take the edge off and make it more live-able. And they are supposed to enable you to function better. I told my pain doc that I was willing to live with a pain scale rating of 5. So, whenever my rating is over 5 we address it aggressively. Most docs will agree that anything over a 5 is unacceptable. Ask your doc if he agrees. If so, then if your pain scores go over 5 he should take action.

Oxycontin will only really last for 8 hours. Many oxy users on this board are taking their dose every 8 hours. You might also mention this to the doc. And, by all means, use anti-inflammatory meds like Motrin if it helps.

    Best of Luck

    steve

Re: Put on oxcontin with percocet for breakthrough, when to take the percocet?

Steve a pain diary is a great suggestion so I started it today, I write down the times for the meds anyway so I will just add to it. I do not like taking 2 narcotics, it seems I am always in a hurry & I am absent minded as it is so I worry I will get confused so I watch my times which can be hard when I am at work.
Thanks guys, you really are a help I am just so unsure about all this med stuff. My doctors may be trying to be careful due to the fact I am asthmatic & not sure if it matters but I am pretty small.
Pepper the percocet reads every six hrs,(before I went on oxy he said if it got nasty take it every 5 hrs), but the doc said no not with the oxy., only for a break through med in between the oxy doses also said he did not want to knock me out with all this stuff. He did mention as a general rule oxycontin is every 12 hrs, is that what yours reads?
I know he does not like to deal with narcotics alot & am surprised he has went this far. I know he knows I am in some nasty pain & the guy(to give him credit) is trying to pinpoint the source, thank god I am getting into the specialist. I am used to being in pain & realize the meds do not totally take the pain away but when this gets out of control I have a hard time breathing, along with some dry heaves & lots of other wonderful stuff. God knows I have done everything I can think of to control it. I admitt I could learn to relax alittle more but I am better then I use to be. I have no doudt these doctors worry about prescribing these meds, but just the reaction at the pharmacy sometimes I can understand (of course not when I am heaving, ha). I will take the least amount of course that works but I in no way want this pain to get out of control, I took the oxy, before even getting out of bed this morning, still had pain but not as nasty as the last few mornings. Pepper hope you are doing well, good luck with the doctor & oxy! please let me know if indeed your script reads every 12 hrs, also what do you find works better oxy or percocet? MRI thursday & back to rehab doc monday morning.
    Thanks, Sammy

Re: Put on oxcontin with percocet for breakthrough, when to take the percocet?

My oxy is written take one tablet every 12 hours.

Tomorrow I have an appt with my GP. Prior to my surgery my GP was perscribing 10-325 percocets. My surgeon added the oxy. He told me I could take the 10-325 every 4 hours if needed.

When I saw my surgeon at my 1 week post op visit; he told me to start coming off the oxy.

I am going to chat with my GP tomorrow and get his spin on the meds. I am really hoping that I can get off some meds but right now I am taking the oxy every 12 hours and am able to stretch the percocets out to every 6 sometimes 8 hours. However, I am still hurting around a 5. I am trying to cut back and take the lowest dose possible. I will post what my GP says tomorrow.

I agree this is alot of meds.

Re: Put on oxcontin with percocet for breakthrough, when to take the percocet?

Sammie, my oxy says every 8 hours. I'm supposed to take 20mg but I talked the pa into 10mg tabs. That way I can take the 20mg when the pain is worse. I discovered something about the 20's. They seem to have an effect on my breathing. If I take 10mg there is no problem other than the pain level is up. When I jump up to 20mg the pain goes down but it's like I have to remind myself to breathe. With your asthma I would make sure someone is watching you for a while.

Re: Put on oxcontin with percocet for breakthrough, when to take the percocet?

Fred that is my biggest concern with all these meds,my breathing. Pretty darn scary. I am really careful because of that. Everytime I have to try something new I get alittle worried, I waited till my husband was home to take the oxy yesturday evening then had him double check with the pharmacist.

Pepper, thanks, now I am reading on the other posts about generics being different, great I put all this crap in my body I want the most relief I can get. Please do let us know what the doc. says.
Thanks guys I will not take anything else other then what I am told, motrin I will try when I feel I need it. trying hard to take the least amount of percocet, only one today, but I am in some pain & it is just getting worse, have to work tommorrow so I hope it gets better.
As for my doctor, look I am grateful he is attempting to help me & is trying to understand this darn pain, I know he does not want to increase or give me anything else & I can appreciate that, he has mentioned my size more then once I am only 5'2 & 106 in weight & he does not want me to be constantly tired either, that is hard because the pain is exhausting to. I just want to get through each day & pray for relief & a diagnosis. I also know any doc. would be hesitant & I am lucky he has done what he has he is not a pm doc & hopefully I won't need one. I already know the next doc is going to complain about all these meds, but if someone would find the problem I would not need them. god knows I don't want to take them & I am not foolish enough to think I won't have a problem coming off them, my husband already spoke to the pharmacy about it. So again thanks, it eases my mind to know I am not taking anything the wrong way. Sammy
