Worried about son!

I have a 12 1/2 yr. old son (middle child) who is very small for his age. He has ALWAYS had anger issues. He gets so angry and frustrated that he just shakes. He absolutely gets beside himself. He has always been very fidgety and always chewing on his fingers nervously. He's afraid of being alone. He sleeps with his older brother. He is very smart and is well behaved besides him constantly getting mad at himself or his sister. I just don't know if this is normal or what. I don't want him to be 20 years old and still like this. Not sure what to do. I'm afraid if I take him to the doctor about this, it will worsen his self-esteem. I don't like the idea of medication either, so I just thought I'd see if anyone else had a child with these issues. Thank You..
