Just Diagnosed

Greetings,

I was just diagnosed with Prostate Cancer this week and have already had my consult with my doctor. My next step is to decide between surgery and radiation therapy.

It seems that most of what I've read concludes that incontinence and ED are foregone conclusions of surgery.

Hi Mickie,

In addition to the questions already asked in the first response, do you have any painful chronic prostatitis (infection/inflammation) or substantial digestive tract problems? The significance is that they can make radiation much more likely to result in bothersome side effects. If not, then the likely profile of radiation side effects, assuming well-done care, is notably lower than the profile of surgery side effects. In fact, well-done, image guided radiation has a rather amazingly low profile of side effects long-term. On the other hand, not all surgery patients experience long-term incontinence or urinary problems, though many do have at least some bothersome issues. These questions have been heavily researched over the years, and research continues as techniques evolve.

Thank you both for your responses. I'm 60 years old. I had three samples come back 3+4 and two samples 4+3 and my PSA is 8.5. I have no pain and no symptoms. Per my PCP and Urologist, my prostate feels otherwise normal upon DRE. Based on my pathology results my Urologist is recommending treatment. We've discussed all of the options including active surveillance. The two treatments that he said are my best options are surgery and radiation but didn't really get on the side of one or the other. He left that choice firmly in my hands. I've read tons. Seems most things I've read say some incontinence and some ED are basically foregone conclusions with surgery. I'm happy to be alive and by no means ungrateful, but not necessarily overjoyed at the side effects if the radiation treatment is equally effective with less likelihood of the same side effects. The Urologist believes they caught it very early.

I am a strong believer in becoming empowered as a patient - doing your own research, and that is what you are doing. You should have an excellent chance at finding a therapy that fits you well and does the job.
