    just shoot me

    hi ,
I had a c6-c7 fusion on November 2013 and have been having some real bad pain still along with some nerve damage in my left arm after the surgery. I have now gone for a second opinion and the doctors told me that the main issue was further up my cervical spine and was totally ignored by this doctor.my second opinion doctor now wants to go in and do a full neck fusion and also fix the half fusion he stated I received .
my recent follow up with the doctor that did the first surgery now says he would like to go in and put some kind of hinge in?? I've been on the computer trying to get the right answer on which surgery to have . I really rather not have the original doctor do this since he screwed me all up and now have nerve damage which I didn't have with prior emg but since had another emg they found nerve damage . what to do

I would recommend you get a third opinion. When there is such a variance in what is being proposed, it is impossible to tell who is correct. Of course, there really isn't just one "right" way to address your issues...even brilliant spine surgeons may disagree on the appropriate surgery.

When you say "full neck fusion" do you mean the number of levels that would be fused or going in from the front side and posterior side?

    Re: just shoot me

I just went last week to see the dr again he I suggestion both front and back surgeries would like to fuse the neck from C2 -C7 . he said he will go in from the front first and then 6 weeks after go in from the back and finish up the procedure. the down fall is that I will only have 50% mobility of my neck .that would be the least im taking it .

I'd defiantly go for a 3rd opinion too like teteri66 said.
I had a 2 level fusion and now I'm having issues with the one above and one below. C-2 to 7 is a "Huge Surgery"

Hope you do go get the 3rd opinion

    Re: just shoot me

And maybe a 4th opinion too . C2 - C7 is a huge surgery. There is a national institute of health/medicine article about the paucity of data in 4 or more level ACDFs so I don't think they are very common with probably good reason. That article suggests severe problems such as multi-segment instability and myelopathy to be likely reasons for these complex and relatively risky surgeries. From what you described that doesn't sound like you although you should definitely discuss with your spine specialists.

I'm a bit confused. Are you saying you have nerve damage stemming from the c6/c7 root the dr operated due to the operation, on or a higher up segment that he/she just didn't do. As previously mentioned, Drs have different philosophies on how to treat multiple segment disease (no surgery unless myelopathic symptoms, only treat symptomatic levels etc). So, not doing a level that later became an issue may or may not be considered "screwing up". Obviously, causing nerve damage when there wasn't any before by doing the surgery is bad (although, a definite, known, risk).

I would get opinions from the best spine surgeons you can find. Often times, those associated with a large teaching hospital are good, at least in my experience. They also tend to have access to new techniques like disk replacement that may possibly be a less invasive alternative to ACDF and certainly to 360 fusion for the segment that wasn't addressed. Good Luck...

I agree to go with a third opinion and see what the procedures need to be done and make a choice then. If you're still uncomfortable, get another opinion.
