    Question Thyroid Peroxidase AB: 100

    Three years ago I had a Thyroid Peroxidase AB lab result come back at 100 klU/L which was flagged as abnormal by the lab. My doctor never brought it up. I only recently became aware of this result because I saw another doctor at the clinic's urgent care and he seemed surprised that nothing had been done about this. I guess I should bring it up with my doctor ?

    Yes, I would. In my case it indicated that I have what is called Hashimoto's thyroiditis
    (an auto immune version of hypothyroidism) and I take daily medication for it, as does my daughter.

    I wouldn't mind at all but it was such a long time ago that unfortunately I don't even remember. I was diagnosed almost 20 years ago.

    I mentioned the high AB value to my doctor. He said that because my other thyroid values are all normal, no treatment is required. I am having a retest anyway since it's been a while.

    The all-important question is whether you have symptoms that can be related to low thyroid function. (Check our "Thyroid Symptom List and Other Information" sticky thread to learn what they are.)

    If you have symptoms and positive thyroid antibodies, it's very unlikely that your other thyroid levels are truly "normal" for your needs.

    If you have symptoms, it's important to have your TSH and free T4 (not total T4). If TSH is above 1.5 - 2.0, and free T4 is below lab median, it shows that your thyroid gland is failing due to the effect of high TPO antibodies. Treatment would be warranted. Many doctors look solely at the TSH measurement between the lab's reference limits and think anything there is "normal". But that's untrue. Sustained levels above 2.5 are not "normal" for more than 95% of the population.

    If you don't have more than six or so of the symptoms on the list and your free T4 is still in the upper part of its range, you probably don't need treatment just yet. But be aware that there isn't much you can do to lower TPO antibodies. Statistically, your thyroid is doomed to fail eventually, and you'll need thyroid replacement therapy. Just so you know.

    I just had my TSH level come back at 6.32, which the lab has flagged as abnormal. It was 3.34 in Sept 2017. If I don't hear from the doctor about this, should I push for further testing ?

    The thyroid peroxidase tests if a positive or negative. Normal people don't have antibodies to their thyroid. After you get out of the normal range (this is the range that the test quantitates) the test doesn't work for quantification. My antibodies are in the middle of the normal range but my body has destroyed 1/3 of my thyroid tissue. I take enough thyroid hormone to shut down my bodies production of thyroid and the peroxidase protein, which stops the immune reaction.

    Your dr doesn't know anything about thyroid if they don't care about the antibodies. A specialist would benefit you.

    Dear ple890,

    TSH level come back at 6.32 usually means that you may need to be taking thyroid hormone. But more needs to be discovered than just TSH.

    Many endocrinologists specialize in diabetes and know little to nothing useful about thyroid. Not that there is anything wrong with that, because doctors are just human and we humans need to have our specialties.

    Thyroid needs are individual and like all hormones, it fluctuates. So we as patients need to educate ourselves about the symptoms and what needs done about them.

    Likewise, there are specialists and general practitioners that make it a point to specialize in learning about thyroid. Those are the doctors that we need for dealing with our thyroid issues. Any type of doctor can specialize in thyroid. We just need to research to find them.

    How I find knowledgeable thyroid specialists is to ask pharmacists at drug stores for names of local doctors that prescribe Nature Throid or other natural thyroid. If they will prescribe those, usually they understand that appropriate thyroid replacement is highly important and individual and that the need fluctuates.
