UTI? bladder spasm?

not sure what is going on with me. The end of Feb, right before my period began, i had UTI symptoms, pain during urination, cramping, urgency ..whole 9 yards. I got some of the otc uricalm to take and had some old cipro. Symptoms subsided/went away. No symptoms the next cycle. This month again, near the start of my cycle it all started again.

I work a temp job, no ins. My mom thought our family dr would at least test my sample and let me know. Nope! the worst of the symptoms, subsided, just frequency, pressure remained. I got busy at work, few days later called my dr to see if they tested it. He would not test it without seeing me. Anyone who has been hit with this economy knows about weighing things .. It would cost me more to miss work, miss carpool then it would be for me to go to urgent care. I went to urgent care a week ago, they detected trace blood in my urine but could not rule out menstrual blood. There were no white blood cells in it, but she said it was the end of the day and of course i had been drinking water all day long. The dr gave me a rx for macrobid & pyridium. I started to feel better, same as before. However, this morning, all the urgency, pressure returned. No clue what is going on. I took a couple of uricalm (otc pyridium), it helped. The antibiotic has run out. The entire time, i've been also taking cranberry caps.

any ideas? if there is a specific test that i should ask for, please let me know. Not sure if another trip to urgent care is the answer or not. That stress of worrying about no ins isn't helping
