No, I am not a big fan of those over the counter creams either. I was referring to prescription grade bioidenticals.
Here is a little cut and paste (tried to give the site name but it won't even take that.....?????)
Estrogen choices.....
Bio-identical hormones by definition means that the hormone is chemically identical to the hormones that are produced by the ovaries.
Estrace® (Estradiol) oral, ESTRADIOL (Estinyl®) , ESTRADIOL skin patches (Alora®, Climara®, Esclim®, Estraderm®, FemPatch®, Vivelle®) contain a natural estrogen female hormone. ESTRADIOL (Estrace®) vaginal cream

Compounded Estrogen
Tri-Est and Bi-Est

Ovaries produce three different estrogens: Estradiol, Estriol, and Estrone. Tri-est is made of bio-identical compound of all three estrogens, usually in the following percentages

estriol = 80 percent

estrone = 10 per cent

estradiol = 10 percent

Bi-est is made of estriol and estradiol only....usually in 90/10 percentages.

Compounded estrogens are available in different forms from a compounding pharmacy:

* creams (skin and vaginal) to rub into thin skin
* sublingual (small pill that dissolve in the mouth)
* troche (gummy like chunk that dissolve in the mouth)
* capsules (to swallow)

The theory is that the natural hormones do much better in the system when it is put into the body without it going through the digestive system first. Meds tend to change in chemical makeup through the digestive system...altering the effectiveness.

Synthetic hormones are defined as hormones that are created by a pharmaceutical company for trademark/brand name reasons to take the place of hormones (or to add to the hormones) created by the ovaries. Chemically, they are NOT identical to the original hormone of the ovaries.

Premarin, Cenestin, Prempro, Premphase

CONJUGATED ESTROGENS (Premarin®,Cenestin™) are a mixture of different estrogen female hormones manufactured from pregnant mare's urine (however Cenestin is made completely from yams and soy)
Medroxyprogesterone is added to either all tablets in the prescription (Prempro™), or it is added only for days 15&emdash;28 of the prescription cycle (Premphase™)

Like I said, I did the compounded route for a year and it worked fine. I also liked the idea that it was made just for me, but also found out that I did just fine on the prescription strenght too. It was just easier and cheaper to switch over. It's also slightly different when you are totally replacing estrogen in the case of surgical menopause versus supplementing low estrogen. I think listening to our bodies is imperative! Just like with TD, having a doc that listens to your body as well as your numbers.

I do find it ironic that with hormones, synthetic is bad (mare's urine) but with thyroid it is considered natural and good in the case of armour! ;)
True not all are, many are. Climara is.
Estradiol is the weakest of the 3 estrogens produced by women.

This is a very good break down of all the different options available .

ESTERIFIED ESTROGENS (Estratab®, Menest®); and ESTROPIPATE (Ogen®, Ortho-Est®) contain estrogen hormones mainly created using plants. These estrogens can help relieve symptoms of the menopause (hot flashes, night sweats, mood changes, and vaginal dryness and irritation), and help to prevent the onset of osteoporosis (a loss of bone mass, so that bones become brittle and easily broken). Generic esterified estrogen and estopipate tablets are available.
