Methadone is a highly effective pain med. And probably the most potent available next to fentanyl. But it is very difficult to dose, especially with those who have used strong narcotics in the past. But it can be done, and has been done for many on this site - including myself.

In addition, as Ex mentioned, it is very inexpensive.

Methadone resides in the same group of opioids as demerol and dextromethorphan (cough suppressant). It is also an NMDA antagonist, meaning it works to block or slow the development of tolerance. Once a patient gets on a stable dose the need to go higher because of tolerance takes much, much longer than all other opioids.

The negatives include sleepiness and sweating. For some these side effects become too much. For others it's not that big a deal. The one other thing is that it can be hard to go off of methadone. What I mean is that if you choose to go completely off of opioids, methadone is one of the hardest. The reason is that it gets stored in the body's soft tissues and is then released slowly over time. So when stopping methadone (and not going to another opioid) the time it takes to get it out of your system can seem awfully long. But, at the same time, the intensity of any bad feelings is lower because of how slowly it leaves your system.

For someone not getting any relief from other opioids methadone is a wonder drug. You just need to know what you are getting into, as with any of the opioids, it has its own idiosyncrasies.
