Spinal Stenosis is a terrible problem for the elderly.
Since the stenosis is generally wide spread up and down the spine it would be difficult to do surgery and make all those vertabrae have bigger inner openings........And spondylosis that nasty seaweed like growth that grows on the vertabrae curving and curling around to make an arthritis like condition and some of these growth are large enough they can actually grow as a moderate sized bone spur and press on nerve roots or the spinal cord...............So as for your 83 year Mother , I think when the doctor told you pain management he means pain blocks...like epidural blocks given specific levels on the spine......While these will not cure......They can help relief the infammation and when the Doctors add some real good long acting numb medication the patient gets and immediate relief that last for quite a few hours up to 16 hours................and during that time the cycle of the muscles spasming is broken so relief may last a week or so....................When I worked in ##### we had so many block patients who were elderly and came in to get their blocks every couple of weeks...............But it was worth it to them ....Giving pain meds in a any dose has a tendency to build up quickly in an elderly persons blood stream causing somulence like a stupor.........so long term use is hard on them.
In all of us who take opiates while we are still young or middle aged Doctors have long known if a person is in SEVERE pain and given doses of Morphine or demerol the med. might stop the pain but doesn't knock them out..........they are in so much pain their body needs this med and they don't ewxperience the drug highs...........So ion severe pain a physical tolerance might build up requiring meds. to be switched .But to worry about addiction rather than treating chronic pain would be cruel! I've had a Fentynal patch, which is a powerful opiate like narcotic for % years.......I started with 25mcg q three days.. and am now on 50mcg q 3 days. The med. works well for me.......I change the patch every three days........I'm glad my doctor did not hold out on me for fear I'd become a drug addict.................After 5 years I'm still having good results on the 50 mcg's and that is a fairly low dose.
Hopefully the blocks will help your Mother and she will only need a low dose of pain medication to work......keep an eye on her and if she appears to be VERy drowsy or confused then take her off the med. and wait until she is alert(or as alert as she was before taking pain meds. )
