one thing my pain doc tells me, is managing pain is like running a car. you want to keep the tank level at all times. when it runs unevenly, the car wont run as well. in other words, he didnt want me to wait until the pain showed up, as it wasnt manageable. he also has me on morphine sulphate 30mg. i take the morphine and then after 4 hours i take an oxycontin. i endured pain that i honestly wished i was going to die. this has made it manageable (i have disk dmg in my Lspine, and arthritis there as well)
also i was on the lidocaine patch. 12 hours on, 12 hours off. i used it for nerve problems from my diabetes on my torso. the pain comes about every 3 months or so, and the patches do the job for that.
i am on pretty much the same meds you are on. i am also on Oxycontin 5/325
