I have a question for you all. It is about Long acting vs short acting. I believe I react better to hydrocodone vs oxycodone.

Why would I go from vicoprofen 7.5/200 4 tabs every 4-6 hours, and eventually I was taking too much ibuprofen and in too much pain during the night and also some during day, so my doc put me on Long Acting O.C., 20 mg, and it was not enough, and then I was on 40mg 2xdaily, I never felt anything but some pain control, wasn't that a large jump, and I still had a medium amount of breakthrough pain? Also at one point before LA, my doctor put me on 2 percocet 5mg and I was to alternate that with vicopfoen. I felt more pain control from the vicoprofen. I chose to just take vicoprofen and alternate them with vicoden if I needed to. So does my body react better to hydro than oxycodone? At first I needed the ibuprofen for pain, and also the inflamation and swelling in my jaw, face, neck and shoulders. I still take vicoprofen for breakthrough pain.

I am now on only 40 mg during the day, I stopped taking the 40mg at night and never noticed anything as far as withdrawals, but do have more pain during the day, and of course especially first thing in the morning. I did notice that if I take my O.C at 8 am, it only lasts till about 3 or 4 pm. I had a dr appt yesterday, and I told him what I had done, he said great, as we had talked about me weaning off LA to see where my pain level was now. I am on Lyrica 150mg 2x daily now, and am hoping that helps with body pain. What should my next refill be for weaning off from 40 mg, should it be 35 or 30?

You all had mentioned something about how short acting meds often work better for people and attach differently to the receptors in our brain. I am excited to hear about the controlled released vicoden, as I will probaly always need some extra pain control.

Hi Kassandra. It seems that you are the perfect example of a chronic PM patient....Fairly complicated case, and one that needs to be tailored to you. What works for you may not for others and vise versa.

This very well could be the case. Also, I think you're right in that the ibuprofen helps as a supplement. Only you know how you feel.

This is because OC is meant to be taken 2x day. When you take it twice a day, your blood plasma levels (BPL) rise over the course of several consecutive doses. LA meds are meant to "level you out" and prevent the "ups & downs" of pain meds...Or in the short acting nature...in & out of your body. This is why you notice the difference in the AM...By not taking at night, your BPL drops significantly.

Ask your pharmacist for the white paper technical data that comes glued to the master OC bottle. I'm sure they will have no problem in giving it you. You can also find it at the Pharma's site. It will explain all of this in detail and even has a graph that shows BPL. For example, in patients who take OC 10Mg, their BPL is higher than a patient who takes 10Mg of Oxycodone IR. Same strength, but OC has a higher BPL due to sustained release and round the clock adminstration.

Very typical. This is why many PM Docs script OC 3x day. Some as much as 4x. My PM Doc insists it's only a 6-8 hr drug, max.

This has me confused. It seems to me that if stop taking OC, your pain and reliance on BT meds will only increase. Am I missing something?

What I mentioned was that when you take meds 24/7, the narcotics permanently attach themselves to the receptors. Conversely, when you take something like Lortab as needed, it doesn't. However, one has to balance the "pros & cons" of each situation. I wouldn't worry about this receptor thing and instead focus on what makes you feel the best and allows you to function.
